North Western Melbourne Primary Health Network

--- North Western Melbourne Primary Health Network

LeadWest has two types of members: (1) the councils of Melbourne‚Äôs west; and (2) other organisations that operate in Melbourne's west.

North Western Melbourne Primary Health Network became an 'other member' of LeadWest in October 2016.

Melbourne Primary Care Network (MPCN) is an independent, locally governed and run, not for profit organisation dedicated to improving primary healthcare in our local communities. MPCN operates the North Western Melbourne PHN (NWMPHN).

NWMPHN has been established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improving coordination of care to ensure patients receive the right care in the right place, at the right time.

Whilst primarily focused on improving population health outcomes, the NWMPHN also works with the primary healthcare sector to improve frontline services.

Collaborating with Local Hospital Networks to ensure better integration between primary and acute care services is also within the remit of NWMPHN.

Interested in LeadWest membership?

Any organisation with operations or interests that are based in Melbourne's west may apply for membership. There is a process set down in LeadWest's Constitution for approval of memberships by the Board of Directors
